644 Improving VTE Prophylaxis in Spinal Patients – a Closed Loop Audit

Abstract Aim Venous thromboembolism (VTE) prophylaxis is one of the key components safe patient care and can be particularly challenging when it concerns spinal admissions to orthopaedic trauma ward. The first cycle our closed loop audit aimed assess safety VTE prescribing in at a district general hospital. current aims evaluate guideline compliance following interventions. Method Quality was measured against NICE (NG89) recommendations as well local guidelines. Patients were identified based on prospectively maintained admission lists. initial presented locally, measures introduced order improve prescribing: staff teaching sessions, nomination guardian, inclusion plans op-notes, publication dedicated addition section into trust principal guideline. 22 admissions. Results Out patients, only (4%) received inadequate which significant improvement compared data (8 cases - 18%). Minor noted three (14%) versus 15 (34%) cycle. Overall, re-audit proved that departmental interventions majority patients appropriate with no or documentation errors (82% vs 48% initially). Conclusions We have demonstrated achieved by means educating introducing simple cost-effective measures.
